Affiliated Collaboratives

An Affiliated Collaborative of the Macomb County Human Services Coordinating Body is a specific project and/or set of services aimed at a specified target population and/or community need seeking HSCB member agency cross-systems coordination, planning, and service delivery.  HSCB Affiliated collaboratives meet one or more of the following criteria: 

 

1.  The proposed collaborative is directed and funded by federal or state public policy through an HSCB member agency.

 

2.  An HSCB Executive Council member agency acts as a fiduciary for funding or as a purchaser of service. 

 

3.  A member of the HSCB Executive Council is directly involved in the the community development of the collaborative and/or seeking private or public support for its operation and continuance.

 

Macomb HSCB Affiliated Collaboratives

Michigan Prisoner Re-entry Initiative is designed to significantly reduce crime and enhance public safety by implementing a seamless system of services for offenders from the time of their entry to prison through their transition, community reintegration and aftercare in their communities. Macomb Veteran Action Collaborative (MVAC)  The mission of MVAC is the development of a “no wrong door” system of care where veterans and their dependents receive the care they need and are referred to appropriate resources. Macomb Children's Healthcare Access Program (MCHAP)  MCHAP is a collaborative, community-based program for children who are enrolled in Medicaid and is designed to reduce barriers to accessing health care at the family, health care provider and health system level.
